Job duties: Manage the company’s home healthcare nursing accounts; Coordinate the coverage of nursing personnel assigned to patient accounts; Review patient’s medical records and collaborate with patient’s treating physician and other specialists to develop, coordinate and monitor short term and long term nursing care plans to be carried out by the assigned home healthcare nurse; Conduct field visits to patient homes in order to assess level of nursing care and to ensure that nursing care plans and objectives are being met; Review nursing notes to assess whether adjustments should be made to the current nursing care plan; Provide weekly medical status updates to treating physicians; Prepare quarterly case reports for review by company executives; Evaluate and assess performance of home health nurse personnel and provide specific feedback to employees; Ensure that home health nursing personnel adhere to company policies and procedures and home health industry regulatory requirements; Review account outcomes and utilize quality assurance protocols to identify and resolve issues and to propose any necessary operational changes to company executives.
Bachelor’s degree (or foreign equivalent) in Nursing.
Position requires Texas RN licensure or a multi-state compact license which grants reciprocity for the RN
license to be utilized in Texas.
